Answer A: What's the best way to avoid slipping on ice?
rubber over-shoes are a frequent answer for this. When I first moved to a climate with a lot more snow than where I grew up I found that they helped a lot. However to really combat slush and dirt I've moved to using boots. Yeah they do have the downsides of time to take on and off, that is true.

Answer A: Which brands make wider hiking boot sizes?
LL Bean "duck" boots worked for me. I had initially ordered a different pair of LL Bean boots but I had to return them because they never "broke in" but I was able to exchange them for their duck boots and I've never looked back (had 'em 4 years already, no issues). I thought they looked a bit 'odd'...
