Question shows activity in list, but no changes found

I've seen this a few times, most recently today with this question. The question list says "last activity about 7 hours ago by Charlie Brumbaugh‭". But when I go into the question, I can't find any recent modifications at all, whether in the question, the answers, or any of the comments. I even checked the history of the question, and that showed no modifications, not even the original content.

What is going on here?

How can we avoid having these old imported questions from popping to the top on the main category?

I think I've read something about activity in questions being related to activity in their answers.

Right, but I couldn't find activity anywhere, whether in the question, the answers, or any of the comments. I have clarified this in the question.
